In your e-mail, please specify your name, appointment date, and the procedure you will be coming in for. If you prefer, you may also bring the physical form with you to your appointment.
*** We require a credit card to hold your appointment. The card will only be charged a $50 fee in the event of a no-show or a cancellation made less than 24 hours in advance.***
